Grapple house rules

I'm curious what house rules people use for grappling. The only one we use so far is that intead of the AoO automatically stopping a grapple if it succeeds (which has always seemed absurd to me --- if Godzilla tries to pick you up and you do one point of damage with a tooth pick, the grapple is stopped) the defender gets to add the damage done to their grapple check to resist the grapple.

So, what other changes do you folks use?

No one in my group grapples enough to have done a homebrew on it. We still have to look up the rules every time we do it.

I've thought about having a Str-based Grapple skill instead of the standard rules, but I had to start my new campaign before I had it thought through completely. The big problem with doing it as a skill is that animals get even more shortchanged than they are already. Many of them would need the grapple skill, but they already don't have the skill points they need for the skills that they have. So that idea is on a back burner for now.

So, what other changes do you folks use?

Well, Godzilla and other huge grapple monsters probably have the Improved Grab quality, or the Imp. Grapple feat. I'm not seeing the problem.
