What about other bonuses and penalties -- e.g., prayer, bless, inspire courage, etc.?

I treat an OGC as an attack roll where "size modifier" is replaced with "special size modifier".

So yup - all of the above, and True Strike as well.

You can "descend" without being prone, though.

Opponent flat on his back; attacker kneeling astride his chest. Advantage attacker... and he's not prone. Disadvantage opponent... and he is.

I'd certainly apply prone penalties to opposed grapple checks.

True, but there are other grappling positions where one fighter is prone and the other is on their knees and the prone figher is in an equal or superior position. For example if the prone fighter has his legs around the waist (or his feet on the hips) of the fighter on their knees this postion is actually slightly in favor of the prone fighter.

It's also perfectly possible for a prone grappler to have a choke hold or joint lock initiated on a grappler on their knees. In this case the prone fighter clearly has the advantage.

In your example might the attacker be pinning the "prone" fighter? In real-life grappling situations being "prone" doesn't actually correlate well with having a disadvantaged position.

Uhm, you mean quickened True Strike during the grapple or True Strike for the next round? A True Strike cast before the grapple would IMHO only be applied to the touch attack, right?

At least grapple nowadays is a way to make a bard shut up.

If you're the attacker, yes.
