Armor maker and mage working together for the fighter ...
Armor maker creates enormously thick and heavy suit of plate armor, granting +20 (or more) to AC if it could be worn.
Mage throws spell to negate weight and mass effects of armor, then throws permanency on armor
Mage throws free movement spell and permanency on armor

Fighter puts on armor, can act and move as if unarmored (a mage could cast spells unhindered and without failure check in 3E in armor.)

If you have done it, what spells or psionic powers were used?

The only spells I can think of that even remotely relate to what you're describing are:

Feather Fall - by definition only works in the air so no go. also, cannot be made permanent

Freedom of Movement - cannot be made permanent

Reverse Gravity - would lift the wearer off the ground. also, cannot be made permanent.

What you are actually describing is essentially the process of magical item creation. (that is how spells that cannot be made permanent get made permanent). Armor of freedom, that effectively negates any and all movement and action penalties associated with the armor should be possible (heck it may already exist). It would require Craft Magical Arms and Armor and probably Freedom of Movement as prereqs.

Best armor for AC I have ever seen is +10, as an exotic armor. If you wanted to change adamantine to grant bonus AC instead of DR, you could bump that up to +13. Add a +5 enhancement bonus and a +5 tower shield, and you would have +27 total. Without additional enhancements your check penalties would be atrocious, and you would have no DEX bonus. An armor with higher base AC would be pretty unbalanced.
