Help - I Need to Find the DMG II NPC Write-Up Block

I saw the Dungeon Master's Guide II format NPC write-up block on the internet a few days ago and I know someone had a link to it, but now I can't find it - google gives me nothing.

Can one of my fellow forum members help me out here?

I've seen some variance in the stat block between DMG II and more recent WOTC books, and Paizo. WOTC uses "Special Actions" instead of "Special Atks" for example. And where evasion goes seems to be another variable - WOTC puts it under Resist, I think, whereas Paizo sticks it next to the saving throws.

Any other differences noted? I'm writing stat blocks for a WOTC licensed property myself so I have to more or less stick to the DMG II version, really.

I use the table in this Word document as a template for the new statblock, adding description of the creature's abilities below the table itself.
