I would just have one dominant feature that potentially threatens an NPC population center (like a volcano, earthquake, falling sky, expanding sphere of annihilation, or whatever) and put a countdown of 3-ish rounds on it.</p><p></p><p>At the end of the 3rd-ish round, disaster happens and everything changes; suddenly, the PCs have to start saving people while Tiamat is still trying to kill them.</p><p></p><p>This all easier if Tiamat gets to choose her battleground. If she doesn’t, try to give her the opportunity to lead the PCs to a better locale.
