How about an "Psychic Storm" in the local Astral Plane? Manual of the Planes has some nice tables that you could roll on whenever they use a spell that has an astral component. Now not only does Teleport not work reliably (always give the illusion of hope), but neither does Summon Monster or Dimension Door. Have the storm last past the current adventure. Make it an adventure hook.

Ye olde Gem of Magical Imprisonment. Basically, it's a deux ex magica item that causes all magical movement within an area or location to end up within the gem. Of course, you'll need to "leak" information to the PCs that the bad guy has made preparations against the PCs, such as hiring some evil magician ally and the PCs finding a similar gem with some souls trapped within. At the end of the climactic fight, the bad guy destroys his gem so he can use his teleport spell -- and ends up within the gem the PCs found.

Liberal use of Illusions.

Big Bad Villan is apparently in room A. In reality he's in Room B wiht an Illusion of some type making the room appear to be room B. PC's attempt to Teleport in to attack the BBV, only to discover they're in an apparently empty room. Empty that is until any traps and/or invisible monsters decide to go after them.

Do that a couple times and they'll rapidly become wary of that tired old Scry then Teleport in schtik.

How do they know what the place looks like? Even if they've seen the place, redecoration may make the rooms very difficult to describe. Ex. move around the tapestries, and do a little painting. Also, why dosn't the bad guy have some sort of protection from scrying? If he can be scried, then he is vulnerabe to this trick. Or if he can detect scrying, he should be able to keep anyone from getting a good look, or spring a trap. When he detects the sensor, he moves his big allies to his current location. I think a leneran pyrohydra is a lot more dangerous if you teleport into a small locked room with it, than at about any other time.

However dimension door might be a bit more difficult, as it can be done without sight.

The well-protected bad guy never lets you scry out his hidden lair. Misdirection spells, screens and the like build up an image of some place that looks right... but it's probably somewhere on the Lost Continent Of Doom that he found once, and will strand the PCs in the middle of a tyrannosaur's breeding ground. I don't have my PHB, so I can't remember whether antimagic shell disallows 'porting in (of course you can't port out). That would, however, be a very nasty way to trap someone on the other side of the world (teleport has unlimited range).

Easy proof vs. Scrying: darkness. The bad guy casts "Darkvision" on himself -- or has a pair of Lenses of Darkvision or an Amulet or whatever -- and, while he can see well enough to get around, no-one can see in.

Alternately, if the bad guy is part demon, he could have the ability to see through any darkness, magical or not. If so, he could easily put a ton of Darkness & Deeper Darkness effects around his lair.

just say "It cannot be done..." and leave it at that...simple effective and brutally honest. Take a hard line glare at them while saying it slowly... The say why say "Such is the way it is...."

i love doing that to my players...they know they are screwed when the DM talks like that..
