Here's my take on the planes, and it can be summarized in one word: infinity. Most alignment based outer planes are infinite, and IMO contain infinite planes as well (only some are known). When you take all those infinities, and fill them with an infinite number of creatures, plane-spawned, power-spawned, and petitioners, etc, you begin to grasp the scale of things.

I would say that gods can comprehend the scale, but most mortals would not, because their scope is limited to the knowable...limited by their own limited knowledge.

As for the other questions, those are best left to the DM, which is why most products detailing the planes have various bits about the DM should customize to fit their setting, etc.
 

Thanks for all the info guys. I'm now reading 2e Faces of Evil and it too has some interesting tidbits about the Blood War.

I also like this idea about blocking interplanar travel to and fro the Abyss. While so quite improbable, it would seem more easily accomplished than the total destruction of the demons.

But I'll start another thread about that right here.
