I guess I tend to be more graphic in my descriptions than most DMs. For an orc in battle I might say, "Your blade slashes across the torso, black-rd ichor runs from the wound. The orc clutches its wounded belly and takes one more swing at you" to mean that an orc is almost dead.
For undead just change "black-red ichor" to "black misty smoke". In short, my undead bleed darkness, when it's almost gone they are almost dead. When you do enough to kill it, the bones in question fall to the ground.
For incorporeal undead, I have them leak black mist and when they die they fade away. Gear has a 50% chance to fall to the ground (and 50% chance to fall to the ground in the ethereal plane).
