How Do You Use Music In Your Game?

I typically find music more distracting then anything else. Recently I've been playing music for a game, but that's because it's a modern game, and the group is in an underground concert. I'm playing the music their characters are hearing ;)

For me, not having music is like watching a movie without a soundtrack. Usually it is low in the background, but combat amps it up (the Gears of War 1 fight music is excellent). I have also had amazing success by letting players hear a distant sound rather than trying to explain to them what it sounds like.

In the Masks of Nyarlathotep i had a woman in labor pains moaning with an echo-effect that ended in a nerve-rattling screech as the party entered the Mountain of the Black Wind in Kenya. Appropriately enough, as the anti-Christ was about to be born nearby, and it was MUCH scarier having them listen to it than just describe it. The only tricky part was Googling long enough to find the right music effect that fit my needs.
