Looks ok, but one question:

For the omega soul, why does your size double, but your horns are still only one inch? So if you are large, and turn huge, you still have one inch horns?

Hmm maybe his horns haven had a growth spirt yet. Okay modify the class (slightly)

he now has 2 inch horns. heh, they have now doubled in size.

As for making it stronger:
Perhaps instead of using the various forms as special abilities, it could be a gradual slippage towards a Half-Fiendish or Fiendish template: after all, that's essentially what the Acolyte of the Skin is, and the Dragon Disciple is a similar sort of thing but angling for the half-dragon template.

Maybe, therefore, the 5th level ability could be one of the above templates and the first four levels either build up to it (stat bonuses, resistances) or enable it to be manifested a certain number of times per day.
It may seem like shafting the Acolyte of the Skin, but bear in mind the Acolyte has other interesting powers and some spellcasting progression.

Go into a rage, a frenzy, and omega soul at the same time. You could also cast bull's strength before this combination for a total of a +20 to a +23 to your strength score. You get an extra attack at your highest bonus. Have healing spells. And don't die until you come out of your frenzy. You could also cast endurance for a total of +10 to +13 to your con score.

While transformed you cannot, doing anything that requires concentration. So no healing spell.

Yes, but you can make a smackdown for any prestige class. Just because a prestige class is 'smackable', it does not mean that it is necessarily overpowered. Indeed, it does not even mean that it is correctly powered. The ranger class is probably (on the whole) a little weak if taken to the high levels, but appears in more smackdowns than nearly any other class (although paladin also comes to mind).

The prestige class has to be taken on its normal merits. Typically, this class would be adopted by a Ftr/Wiz or Ftr/Clr, or possibly a Brb/Sor. In this context, it is not so powerful. After all, one needs to be at least 10th level (Ftr7/Clr3) and even then has to buy skills cross-class and take a near worthless feat. Given these stringent requirements, I would argue that this is not a very powerful prestige class.
The only really useful ability is the Omega Soul, which can only be gained at 15th level, used once per day and has a hefty XP tag attached.

Therefore, it needs to be made stronger- some sort of drift towards a fiendish or half-fiendish template may be of the order.

Actually the DM also told me it seemed kidna weak, I got to change to to +12 strength and +8 consitution. For Omega Soul.

Dark essence now gain +6 strength and +4 consitution.

Lost soul now has +4 strength, and +2 consitution.

The modified version sounds closer to a respectable prestige class.
