Look up Empire of the Petal Throne. MAR Barker did indeed give TSR the rights to publish the Tekumel setting using the OD&D rules in the early days. IIRC, it was actually published before Greyhawk was.

Since then, Professor Barker has had Tekumel published under a variety of rules systems, and it was most recently published by Guardians of Order, using the Tri-Stat system.

Edit: After some quick research, stats were percentile, not 3d6. That's a key difference from the start.

If we break it down that much then we'll have to list every alternate plane (like those in Q1).

A reasonable qualifier is probably that the setting must have had either a setting book, or else have appeared in more than one adventure or series of adventures exclusively.

Yep. Order of the Petal Throne was not a D&D setting but a standalone game published by TSR. It used a much different system than that used in Chainmail or OD&D.

[Edit: I owned the original TSR EPT game for years.]

Not too hard since so far only Judges Guild & Kenzer have been 3rd party companies ever been permitted to produce D&D settings (with Wee Warriors not producing any settings).
