How much playtesting does published content see?

I'm curious how much playtesting professional published content gets. Just to clarify that: I'm talking about content for which the author was payed to write, and which the consumer is expected to pay for (not counting free previews and such). I'm particularly curious as to the experiences of people publishing content for D&D 3rd and 4th edition, but really experience from any game would likely be helpful.

You don't need to out any companies you may have worked for, though it would be helpful if you identified them in very vague terms (e.g. "a small PDF publisher," "a large 3rd party company").
