how to measure time

Does anyone know how to measure time in D&D? I've seen that spells, auto-healing etc take in account time, but I can't find rules for establishing how much time has passed.

And if you are outside the combat round system (as in not in combat) then it is up to the DM to determine how long it takes to do things - pure judgement (and using real time is almost always a bad idea ; ))

What I mean is: (quotes from Healing- Combat chapter of PM):"Natural Healing: With a full night’s rest (8 hours of sleep or more), you recover 1 hit point per character level" AND "If you undergo complete bed rest for an entire day and night, you
recover twice your character level in hit points". So how do I know when the "day and night" rest begins and ends? when the 8 hours are complete? when can a rest begin- even when fighting or what?
Thanks for your time

Well you can't get complete bed rest if you fight. I'd rule for the day or night thing that someone would have to spend at least 24 hours to get one set of 2/level in Hit points healed for comp. bed rest.

Entire day and night = 24 hrs. Rest begins when you actually start resting, i.e. not doing anything and definitely not engaged in any strenuous activity whatsoever.

That's not what I meant. What I meant is: isn't there a kind of built-in CLOCK for this world? Eg the spell "Alarm" says: Duration: 2 hrs/level. So I cast this spell. How do I know when the 6 hrs have passed??? Is this the DM's decision?

I say spellcasters know when the duration of their spells run out. And yes, of course it's the DM's decision when the 6 hrs have passed.
