D&D 5E I know nothing about "First World," but here's an idea I'd want to play...

Inspired from another thread...

A primordial world ruled by titans and dragons, both of whom created offspring as a kind of servant class: giants by the titans and the various lesser draconic beings by the dragons. The titans and dragons have an ongoing war that has gone on for thousands of years.

Meanwhile humans are born. Not sure how - maybe one of the titans went rogue, ala Prometheus. Yet now wee little humans live in a world dominated by titans and dragons, sort of a D&D version of "Kaiju world."

Perhaps elves have arrived from space or the Feywild, and teach humans various arts - including magic. Dwarves exist as the Promethean titan's earlier attempt (shades of Tolkien here). Gnomes hitched a ride with the elves, and halflings...well, not sure how'd they fit in, if at all. Maybe they are the descendants of a group of humans that survived the Titan-Dragon War by hiding out in small places.

So it is a rough world, with the Titan Empire on one side, and the Dragon Kingdoms on the other (Dragons can't agree upon an imperial hierarchy, but instead rule their own micro-kingdoms, perhaps based upon color...so the biggest, baddest blue dragon--for instance--is the king of all the blues). Humans and other races try to carve a place out for themselves, but live on the fringes, constantly hiding from the dragons and titans.

Oh yeah, there are ruins and such, which is part of the mystery of the world that the PCs might figure out. But evidently humans have existed before, but were wiped out - maybe several times.

Oh, and the Tarrasque was a war creature created by the Titans to fight the Dragons, but it went wild.

And so forth.

the ruins could be really alien as aboleth would certainly be around at that point.
