Ack, you're right, I misread, heh, it is ranged attacks.

During game play those are the same calls I've seen other DMs make too, makes sense when a dragon is chewing on a halfling, poor halfling :D

If a monster takes -20 on it's opposed grapple check, it is not considered grappled, i.e. it still has it's dex to all attackers.

I'm afraid so, taking the -20 eliminates the penalties of being in a grapple allowing her to otherwise fight normally but she is still in a "grapple" with her target. By definition this means her target retains dex bonus from her attacks, now other opponents however would be able to target her with sneak attacks
