D&D 5E Is the Druid separate from the Wildshape?

Hi, question on Druid Wildshape.What happens to a Druid's actual body when she Wildshapes?More specifically, we know that wounds from the Wildshape do not carry over if the Druid reverts by an action as opposed to being damaged below 0.But if a Druid is say poisoned or bleeding before shifting? Does the Wildshape appear at full life but with those negative ailments? Or do those ailments only affect the Druid when she reverts?

The druid is the druid. So if there is a condition on the druid when they wildshape it stays when in beast form. Prone druid wild shapes, still prone when he is a bear or an eagle. Same with poisoned, exhaustion, blinded, or frightened. Nothing in the description of wild shape says it removes conditions or suspends them.
