Allows you to either make an additional 5' step OR make a 10' step but treats it as a 5' step? I could have sworn I have seen this printed somewhere before. Maybe not a WotC or 3.5 book though...

In the Expanded Psionics Handbook (and the SRD) the Elocater prestige class gain Capricious Step an ability that allows it to take a second 5ft in a round. Also in the Arms & Equiment Guide there was an item that let monks take 10ft 5ft steps. I think that may be one or two other things in WoTC books that grant the effect you are looking for but I do not remember where or what they are.

The Sidestep feat from the Miniatures Handbook lets you make a 5' step after a successful AoO (once/round).

I also seem to remember something about 10' steps in the Oriental Adventures book. [searching...] DC 40 tumble check to make 10' step.
 

There's a monk-only magic item called Training Dummy of the Master (or something similar) that after using it for a while lets you move 10' as a 5' step thereafter. I believe it appeared in the Arms and Equipment Guide.
 

In Sword & Fist for 3.0, the Master Samurai prestige class acquires Supreme Cleave early on, allowing him or her to take a free 5-foot step after each cleave/great cleave attack (or maybe it was just before each cleave attack).

It is called the Sparring Dummy of the Master and it is indeed in the A&EG. It works like a manual or tome of stat enhancing (in that you have to use it over a period of time).

and indeed, the character must have one level in monk to use it.

Monk Elocators are fun... :P "I'll take a 10ft step, full attack... then take another 10ft back..."
