I might add something about AOOs, like a peson with a Pike is effectively a ranged opponent and does not threaten any squares.

I think they'd still be able to make AoO against people they threaten. Obviously, someone with a 20 ft spear can't threaten a square that is 5 or 10 feet away.

Whether it is Exotic or Martial should depend on the setting, not the game balance. For example, in the PHB, a repeating crossbow is an exotic weapon, even though it is not any more difficult to use than a simple light crossbow (point, shoot). The difference is the cost and availability and technology involved.

More poignantly, notice that all the oriental weapons in the PHB are Exotic, but in the Oriental Adventures, they are Martial.

Notice too that dwarves in 3.5 get to use the urgosh as a Martial weapon because it makes logical sense for their culture, not because 3.0 dwarves were "too weak" and needed to be boosted up.

In the Untamed Legends that I'm working on, there is something called a Dragon Spear which is used for fighting huge creatures (which are fairly common in the setting). It triples the user's reach, rather than doubling it, as a normal long spear would (3.5 PHB page 135).

It does 1d8 x3. It is an Exotic weapon because, although huge creatures are fairly common, 20 ft spears are still a rather new idea and are not common since most people don't even try to fight with huge creatures. They prefer to run and hide.

One way to balance its historical use as a formation weapon and its potential use as a character's main weapon would be to have it function as a 15' reach martial weapon which disallows (or suffers a large penalty) on Cleave, Disarm, Trip, etc. However, a character with exotic weapon proficiency (REALLYlongspear) can use it without penalty, and can shift his grip to wield it as a 10' reach weapon as well. This would definitely be more suited for high fantasy or wuxia settings than 'realistic' ones.
