Looking for a Player Survey/Questionaire

So, I'm looking for a thing. I've tried searching for it, but I haven't had any luck. I apologize if this this request might come off as pretty vague.

Several years ago, I participated in a 4th edition D&D game. The DM sent me something akin to a "player preference" questionaire/survey. I emailed him about it, but he wasn't able to find it in his saved emails, but did recall finding it here at enworld. I don't have very many details on it. He would have found it before mid-January 2011, and he I'm pretty sure he sent it to me in a word document. I recall it was primarily a multiple choice document, asking me things like what kind of games I enjoyed, for example: political intrique, dungeon-crawling, roleplay-heavy, etc.

I'm sorry I can't be more explicit with details, but I would greatly appreciate help finding that document or something similar.

These are generally made by individual GMs to make sure their GMing style and your playing style would work well together. Each person who runs RPGs and does this has their own, so while questions may be similar, the chances of them being exactly the same is small.

If on the other hand, you're talking about a quiz that identifies your play style, there are plenty of those too. At the moment I have no links to any.

Thank you both for your quick replies. As it turns out, my old DM was actually able to find the original email, by changing his search terms slightly.
