Looking for advice/ideas on running Kingmaker

They used their normal Kingmaker characters when I ran it. Just integrated the mods into our campaign and it made sense. If they died in those areas, then they died in those areas.

As for leveling, with the APs I find it much easier to just use the assumed leveling noted at the beginning of each module and tell players to level when appropriate.

KM was my first adventuring path and I really wish someone like IronWolf had simply told me to handle xp that way.

And as for the other modules, it's your (your DM's) game, but those modules interjected into KM will add to the flavor.
