I'm about to use a module with a trap that may cost someone a hand in my game, and I'm deciding how to handle it given the fact that at this point the PCs, levels 7-10, have been all over the known world and have never met a cleric over 10th level. They know of one 10th level cleric, but no one else comes even close.

But then, I'm infamous for my maimings of pcs. Why, there was one guy who lost his leg to crits so often that it became his theme song-
"Drelvin, he went to the Underdark-
Drelvin, he lost his leg!
Drelvin, he fought some devils-
Drelvin, he lost his leg!
Drelvin, he founght an undead lake monster-
Drelvin, he lost his leg!"
Etc.
Anyhow, if you chop off his hand, you've just created a fantastic opportunity for adventure. Now they have to find one of the world's only 3 13th level clerics (or however many). Or hell, maybe they will need to somehow help one advance to 13th level...
Edit: and there are grafts and prosthetics, too. A prostheticist can be really cool, especially if the campaign allows for specialized prosthetics.
