Said house rules are a bit of a monstrosity (but nonetheless, a collective that functioned well in practise), so I won't go into them here and now.
Heh. I seriously doubt they were more of a monstrosity than what I used last time:

Still, those rules worked well in practice too. Now that I'm on hiatus while another guy in our group runs Age of Worms, I've been thinking about it, though, and I'd like to streamline the rules up a bit--that's why I'm going with d20 Modern +d20 Past.

I guess that's not necessarily more streamlined; it is focused on fewer sourcebooks, though.

My group has been having fun with Iron Heroes. Instead of having reams and reams of houserules on magic, spells, alignment, character progression, etc, I have one book that I can refer to. Iron Heroes does have some rough patches, and you need to be aware that IH PCs are more flexible and durable than their vanilla D&D counterparts, but I think it's a good starting point.

I'm running two low-magic d20 Modern games. The main supplements I use are Blood & Fists: Master Edition, and Blood & Guts 2. It's a special ops themed campaign that draws elements from CoC and Delta Green, and the X-Files.

I can't quite tell if you want more recommendations on how to run the low magic portion of the game, or the grim and gritty. Though the two often overlap, they are not the same thing.

I use a Fate Points mechanic similar to the one in Conan OGL.

I use the expanded Action Point rules from the d20 SRD, with the caveat that the feat emulation only works with feats from the d20 Modern SRD.

I also give out feats and training based upon missions for free.

Everyone seems satisfied so far, though the two campaigns are only at 4th level each, now.

Bo9S as its own setting is a perfect Grim and Gritty low magic campaign

Wait..Deja Vu...Did I already post that?

Well then, Swashbuckling adventures is another personal favorite.
