The RPGA's Living Death campaign uses a version of the old Masque of the Red Death rules, converted to 3E (then 3.5) by the Living Death campaign staff.
Although it's written with the Living Death campaign in mind, the rules are available for download for anyone who's interested.
Check it out at www.livingdeath.org -- you'll want to download the "Living Death Campaign Sourcebook D&D 3.5", which is linked about halfway down the page.
In playing LD over the past two years, my PC (a British physics professor) has met mummies, lycanthropes, liches, etc...as well as a few historical figures, including a 5-year-old JRR Tolkien.
Living Death is right up there with Living Force as being my favorite RPGA campaigns.
