So, how much would this cost under 3.5 magic-item creation rules:

Periapt of Protection from Poison
The wearer gains a +4 luck saving throw bonus against any type of poison.
Caster Level: 5th; Prerequisites: Craft Wondrous Item, neutralize poison;

Backstory: This is the exact same effect as the 3.0 Periapt of Proof against Poison which had a cost of 4000 gp. I am converting a published 3.5 adventure and I don't feel that the 3.5 Periapt which has a price of 27000 gp would be appropriate.

You didn't mention that the 3.5 version makes you immune to poison (although upon donning it any poison in your system runs its course).

Lets start at table 7-33: Estimating Magic Item Gold Piece Values. Save bonus(resistance) is bonus squared x 1000, and Save bonus (other) is bonus squared x 2,000. Also this includes all saving throws, so we'll have to reduce the price for only affecting one save, and then reduce it again for only affecting a part of what fortitude covers.

+4 Luck bonus is 32,000

1/3 of that being fortitude, we get 10,650 (roughly). Seeing as fortitude can help against spells, poison, disease, and other misc. things I would reduce the price by half. So about 5,300 for +4 luck bonus vs poison. If it was a resistance bonus it would only be 2,650. Or it seems you could use the 3.0 version+value with no problems.
