Another thing to consider would be reviving the character. Lifeguards revive nearly drowned, unconscious people every day. Another possible HR could be a heal check to stabilize the character before death.

Hmm, never really saw it that way. I always thought the second part of the description was sort of re-emphasizing the consequences if you tried to breathe. It seems odd that if you just cough and spit, nothing bad happens other than you can't do that, but if you hold your breath, *then* try to breathe, you die. But I can see where you're coming from. Then again, I just Fort save Miasma and call it a day. Oh well, thanks for the clarification.
