It is the game design. And in its design it actually seems ok (if leaders can shore up defenses well enough and the defender does its job right), and hit are more or less expected within the boundaries of this game. Even FRW are ok if you distibute your attributes in a conservative manner and do something for those defenses...

It is a design shift from ADnD to 4e to make hits less dangerous in general and allowing hp to protect you. IMHO in this process status effects were underevaluated. Thus you have to be careful as a DM utilizing them.

I would have favoured lower attack bonus for monsters. I also would have favoured monsters with higher AC and more hp and monsters with lower HP and higher AC. But in the end, with the right mixture of solo, elite and minion of higher and lower levels you get the right mix of those monsters. And in the end attack bonuses are ok.
