Most freakish template creatures?

I haven't really gone nuts with the templates or anything, but the creature that most scared the PCs in a long time was a Vampire Medusa that I created.

The most extreme template I ever considered was:

Half-Fiend Corrupted Corpse Evolved (Twice!) Spell-Stitched Human Fighter 12

What stopped me from ever statting it out was the simple knowledge that I'd never GET PC's strong enough to fight the thing.

Also, I once tried to figure out how to make a Vampiric Spell-Stitched Swarm Shifter Advanced Orca.

Think of it! A killer whale that drains blood, fires off spells, and bursts into a massive swarm of leeches!
 


One of the minor Big Bads in a game I ran (the players fooled around with a Deck of Many Things and he was the Emnity) was Ravanaya, a multiheaded half-fiend greater pseudonatural colossal spider. I liked him a lot.

PKitty did a grell monk for the old Dragonsfoot Editions War forum. it had a bunch of stuff that boggles the mind, but is legal/"official" for the newer editions.



edit: the most recent templated monsters i have created were the Beatles... John, Paul, George, and Ringo... fire beetle/ remorhaz hybrids

Well, I had one player who wanted to apply the "Dead" template out of Portable Hole Full of Beer to his character... ;)
 

I love templates! But some of my favorites are my own homebrewed ones.

I really liked the 4-headed corrupted bile chuul. TPK in waiting- in retrospect, the CR shoulda been higher.
 

Fiendish, feral (breaking creature type rules), monstrous adept (homebrew template), 5-headed lernean cryohydra?

Not technically a stacking of templates, but there's Animus Anthraxis, of a race that is half-ogre mage, half-blue (goblin with psionics), and Animus had the half-fiend template and Fighter/Wizard/Spellsword levels.
 

Pseudonatural half illithid yuan-ti anathema. I loved it, it was my widdle biddy pet.

But i think im going to have to steal Jurgens little girl with 3 eyeballs per socket idea.

Only problem here is that I generally don't bother adding proper templates to pre-existing creatures so much as I just create creatures and give them stats without regard to what 'templates' they might have. As such I'll leave out that whole host of wierdness.

Among actual templates though, I had a half-fiend death kiss beholder-kin. I also had a flock of Half Mineral Quasielemental Nycaloths, some Half Lightning Quasielemental Arcanaloths, and some Half Ash Quasielemental Mezzoloths.

For just -odd- creatures obviously created by something like a template of sorts, I had some creatures that amounted to half-fiend gibbering orbs coordinating in a sort of limited hive mind under the ultimate control of the arch-yugoloth who created them.
