The Bo9S has several variations on this idea, I think all of them are part of the Tiger Claw discipline.
Bo9S also has the Snap Kick feat which gives you an additional unarmed attack when you make an attack, a full attack, or even a Strike.

Also: Iron Heart & Desert Wind both have multiple-attack Strikes.

Tiger Claw clearly is the place to go if you want two-weapon fighting - there's extra attacks, rend (that gets better the more often you hit) - but other disciplines also have some good stuff, like Diamond Mind's quicksilver motion which lets you make an extra move action. Since you usually get manoeuvres from two disciplines (three for a swordsage), you can combine them with Tiger Claw.

A swordsage or warblade Tiger Claw and Diamond Mind can do some nasty combos. A warblade's attack bonus will help, but the swordsage can use insightful strike (the class ability, not the manoeuvre), and on 20th level, they get the ability to use two boosts at once. A swordsage with TC and DM with two weapons, using Time Stands Still together with both Girallon Windmill Flesh Rip and Raging Mongoose can make something like 16 attacks (and if he has two speed weapons, this should increase to 20), and get extra damage to everyone he hits more than once (and of course, insightful strike will let him add wis to all of them, maybe even dex, too, if he adds some shadowhand to the mix)
