Natural Reach and Weapons?

First, let's say a monster has a reach entry in the MM of 10 feet. Does this mean it can't attack someone in a square adjacent to it? Or does that only apply to reach weapons?

Second, let's say a monster with a 10-foot reach is using a weapon with 10-foot reach. Does that mean the monster can only attack something with the weapon if the target is 20 feet away? Or is anywhere within the 10-20 foot range eligible?

That only applies to reach weapons.

He can attack creatures which are 15-20 feet away, but not those who are 5 through 10 feet away.

When determining how reach weapons work, do the following:

Double your normal reach. Anything greater than your normal reach but less or equal to than your improved reach can be attacked. Anything less than or equal to your normal reach cannot.

This is assuming of course that it is a normal reach weapon sized properly for the creature. A large creature with a large spiked chain, for example would let the monster hit anyone within 20 feet.

The rules are at present rather murky as to what happens when you mix reach weapons whose size doesn't match the creature wielding them. This could be either up a size (courtesy of the monkey grip feat) or down a size or two (a large creature wielding a medium or small longspear for example). The house rule my group has used for mismatched reach weapon vs creature sizes is that the weapon adds the reach of the creature it is sized for to the minimum (0') and maximum reach a creature has.
