Do you have any Planescape material? They could be a lot of help with this...
Apart from the Campaign Setting, I recommend Faces of Evil: The Fiends and Hellbound: The Blood War - both are great for fleshing out villains for truely epic campaigns. And it is hard to imagine something bigger than shaping the course of the Blood War.
And if you want to "power up" the fortress of a villain a bit, I recommend my ideas on Nexus Towers

Nope. The new Manual of the Planes comes fairly close in detailing the basics of the Outer and Inner Planes, though. The only notable changes from the basic Planescape cosmology are that the Astral and Ethreal planes are linked to all other planes, and that the plane of Shadows was made more important (it used to be a demiplane in Planescape...).
