No, Toril, Oerth, and Krynn are not linked though the great wheel in my campaign. However, it is possible to travel from the Cosmology of Toril to the Great Wheel (and vice-versa) by traveling through the Plane of Shadow, but those who undertake such a journey are rare.

Had to go with "other." I don't run games in either Toril, Oerth, or Krynn, and have no plans ever to do so (except possibly Oerth, if I ever get around to doing my "Campaign of Classic Modules Updated to 3.5." :))

However, when I run Planescape games, I tend to allow access to all three of them, if any of the players want to come from/go to them. So in those circumstances, they're still linked--if not by the Great Wheel itself, than by whatever connects the Great Wheel to other cosmologies.

(Of course, if you do have them linked, you have to watch out for those players who always want to try the "Buy lots of steel on Toril, sell it for gold on Krynn, use that gold to buy even more steel on Krynn, etc. ad nauseum" trick.)

My homebrew is not connected to any of them, so I can't answer the question. Maybe they are, maybe they aren't; the PCs will never know. And they'll know even less that they exist.

Phlogiston all the way, baby!

It's been many long years since I last gamed in any of D&D's major campaign worlds, but at that time they were definitely linked via Spelljammer.

Here's to having the worlds be connected, as they should be!

For me, there never was and probably never will be a multiversal design better than the Spelljammer/Planescape one, where all the worlds and planes were part of a single multiverse. I miss that intensely, and will probably carry a torch for it forever.

They are in my current homebrew but for the moment its largely irrelevant since the Unbounded World (High Level Period) people don't yet have the knowledge to create Spelljammers and the people of the Sealed Lands (Low Level Period) have even less knowledge and wouldn't have the power to create the ships anyway,(If you're in my PBP games don't read this :P)
though I have considered having an encounter with a crashed Spelljammer.

I answered that they are connected for this same reason. In my old campaign I used the three base settings (Greyhawk, Dragonlance and Forgotten Realms) as alternitive realities for the prime. When my player's travelled from one alternitive prime (they only did so once, and they were very quick to return) they found out that common isn't the same on these three worlds. It might carry the same name but it's a very different language.

Ravenloft stands appart from everything (I don't know Mystara, Athas and Birthright so I can't comment on them.) since it's just an extra-dimensional "prison". Extra-dimensional means that it has absolutely no connection to the ordinary world.
