More spoilers:

An excellent place to put the solution to a second mystery is in the unexplored caves near the idol. An alternative spot is to tie it in to the young lady near the pool. Either one of those would easily meld with some fun plot hooks, especially the latter.

Crothian, the halfling party imc left several people with certain, um, head issues undelt with. Who knows what could come of that sort of thing?
 

Cool, well, we doidn't get to the module tonight. I had them find a copy of a journal in a friends stuff as they helped him move. That sparked interest and they did some research and got some very good background info. However, they spent some time dealing with other plots, but next week we should start it as the town lies on the way to a place they want to go. I got lucky with that.

Put simply - the old mine used to be in the uppermost face of a crevace of some kind, with the dragons lair hidden at the base. The reason the mine became abandoned after the dragon was defeated had as much to do with the earthquake summoned by the higher-level cleric in the battle, trying to close of the dragons method of escape. The crevace is now a narrow and unstable rift barely a few dozen feet apart. The old mine access is still high up, but the goblins have rigged deadfalls and rockslides to capture anyone who tries to fly to the base of the rift and enter through the dragon's portal.

The only real change needed would be to make the climb through the waterfall down, instead of up, and think of a good reason why the fey is at the top.

Thought I would answer this question from the perspective of my co-DMed campaign. This is a homebrew where much of the world is unexplored, or has not been explored for some time.

Be default, because no one had brought it up, psionics were not known to exist in our world. I set OSM on the outskirts of our known world, and tied it in with an ongoing plot to get the PCs there.

The events at the end of OSM, with the PC's success, triggered a psionic-based "explosion". Now, the people of Bellhold are predisposed to develop psionic abilities, and a high percentage of the population will be psionic the next time the PCs show up there.

Perhaps that could happen when the sequel sees print? (hint, hint)
