Old customer response card (1994)

I just received a package of old second-hand AD&D and D&D 3E products I'd ordered about a month ago. (Shipping from the US can take some time). It was a pretty big package, and amongst all the stuff was a shinkwrapped copy of Marco Volo: Journey. Most SW material is worth a bit - this one isn't and rightfully so. The shinkwrap was actually too tight and deforming the book, so I happily removed it and discovered this old customer response card from TSR in 1994 within:
