The first edition did. The second edition - published by Bantam Press through arrangement with SPI - was just the rulebook plus the starchart.
Incredibly, I did buy the GM Screen for it as well.

I've got the book packed away somewhere. I believe I lost the starchart before doing so, unless it inadvertantly got packed away with the later spacemaster books instead.
It was one of the first things I got my hands on, so helped fire the imagination and hooked me into RPGs.
Took me forever to get Delta Vee, and then I was disappointed because I wanted to truly create whole lines of ships from scratch and at best (IIRC) it just gave you a couple more pods you could swap in place of the few you got on those few pages inside the Universe rulebook.
