I'd expect you'd give them a minor penalty to all physical tasks until they're used to it, and of course not let them use the missing arm for anything.

Do you really need more rules than that?

Arravis, there are no rules for a one-armed character.

In Greyhawk or the Forgotten Realms: get an Arm of Nyr.

If the Arm isn't available, pay for a casting of Regenerate.

In worlds where neither is available, just role-play the lack of one arm.

Rules-wise, the only core rules I can think of are that he couldn't wield a greatsword (without the Monkey Grip feat,) or fire a bow.
 

You want rules for a one-armed character?

You suffer a -4 penalty on all Climb checks. You suffer a -2 penalty on all Craft, Disable Device, Escape Artist, Forgery, Heal, Open Lock, Sleight of Hand, Swim, and Use Rope checks. You suffer a -2 penalty on all Strength-based checks. You are unable to wield any two-handed weapons.

These were gleaned from the Variant: Damage to Specific Areas in the DMG. I just applied the penalties for damage to a hand and damage to an arm, doubling the ones that overlapped. However, I removed the penalty to attack rolls (which would have been a -4, since both hand and arm suffer from it, doubling the penalty) since you would be using your other remaining arm.
 

Yeah, no shield, no TWF, no twohanded weapon...

I'd give a +2 strength though. Onearmed dudes tend to be horribly strong with their one arm.
 



Sounds like my favorite idea for a GURPS character: A one armed, ambidexterous guy.

Now that I've got that out of the way, I think a one armed character could use Heal without penalty. It may take lsightly more effort, but i think he could do it just as well.
