HOLY COW!
V IS A MAN! Check it out. The last panel. The imp says "his dim-witted bird" implying that V is a man. Do you think this was a slip by the author or that the demon knows V's true gender?
 

The author (Rich) has stated that each character refers to V using the gender that character perceives/assumes/guesses V to be. Apparently, that imp thinks of V as a male. That does not mean that V is a male.

I thought that "he" was a default in cases where the exact gender is not known. The alternate, "it", is not appropriate for people. English doesn't have a gender neutral pronoun to use. (This can lead to all sorts of discussions about sexism in language. But let's not go there.)

Sort of. But you wouldn't refer to Pat as "he" if you didn't know whether Pat was male or female. You'd probably refer to a person by his name until you learned his gender. (See what I did there :-)
