Well, I think John was trying to figure out to stay or go and how best to protect earth from the Peace Keepers. I also think the 'old woman' was Scorp (inside the mind) now the question was all that in his mind or did it really happen.
I think the worm hole that took Moya was created by the aliens that crashed into her in season 2.
I will refrain (for the moment) from giving too much in the way of spoilers. However, the old three-eyed lady was not Scorpius/Harvey inside John's head.
The three eyed lady was real. She was recognized by other crew members. The episode was a bit non-linear because Johnny-boy spent a significant time under the effects of mind-altering substances.
A rather unexpected season finale. Confusing, disjointed, yet one more person frelling with John's mind. I found the End of Lexx far more interesting, but I'm still very much looking forward to the next season of Farscape.

Oh well....at least this one had some amusing moments, but it was a bit too much stream-o-consciousness to stand up as a really good season finale. The pacing was just off for the entire episode, and the wormhole bit at the end was the most mis-paced element of them all....
While I am not sure who created the wormhole at the end, I AM sure that it was entirely appropriate. John's indecision finally bit him in the ass (more than once) and now he doesn't have either bone (taking the analogy from the ep). And how can you not think this was the best episode of the four after the old woman whispers that Aeryn...
[SPOILER AHEAD]
"...is with child."
I personally found this jaw-droppingly good. Sorry to see I'm the only one who thought so so far. The characters were all on top of their games and it was obvious that it was one of the big-wigs (David Kemper in this case) who wrote the episode. I embrace the next season with open arms, especially when it will be paired up with Stargate!!
