I work in Market Research ( and have since '95 ) and see that you have done may things right on your survey.

You have done many things wrong too ( meaning you won't get as much out of this study as you could have ). I'll spare you the critique, as it is probably too late to make changes to the instrument. Feel free to email me ( see my profile ) if you would like to get more feedback from me.
