Um, page three is blank. I'm guessing that is where your OGL would be... because you certainly wouldn't forget to append a copy of that, would you?

A nice read. Some formatting-related comments ...
I prefer the sneak attack entry to indicate the total bonus, not the bonus added (which is what I assume you've done on your chart). I guess I just expect it, since that is the way it appears in other sources. I might point out that all other game mechanics (such as BAB, Saves, etc) are the total, not an amount to be added.
I prefer fully-justified columns.
I'm not familiar enough with the setting to render an opinion beyond that I'm afraid, but I always like to see people putting out new content.
