plane shift + astral projection = no-risk fights?

Here's my situation, I'm running an evil game with two death knights in the party (lvl 10, going to lvl 11 next game) with their signature nightmares.
Component 1: The nightmares have astral projection at will at caster lvl 20, which means they can take the entire party with them.
Component 2: the cleric in the party has plane shift, which allows her to take the entire party to some other plane.

So, the party can go to some other plane, find a safe spot, and astrally project back to the prime material plane for any big fights at no risk, since if they "die" they just get kicked back to their own bodies, without even losing any equipment! To keep this in check I've informed the players who have characters with planar knowledge that there are things in the astral plane that like randomly going around cutting silver cords. And a side issue, I've ruled that it doesn't make since to astrally project onto the plane that you are currently on, along with plane shifting.

Anyone dealt with this situation, either with the plane shift + astral projection, or just with astral projection being used to handle fights without a high risk of getting killed?

Also, what seems like a resonable mechanic for having the silver cords cut? I was thinking a % chance per hour or day for it to happen. (gith wandering by, or one of the nice epic mosters who eat silver cords)

Targeted or area dispel magic, greater dispel magic, mage's disjunction, or dimensional lock will make life for your party very difficult.

The baddies may be surprised once, but they'll be prepared the next time...
