I'd suggest that to get some kind of central leadership to the city whilst allowing it to be easily 'plugged in' to an existing campaign that you make the Leader the 'Provost of Merchants' operating under a charter from the ruling authority of the land (whomever that maybe is the DMs choice). The Provost would essentially be Mayor with the power to tax, defend and regulate the Port on behalf of the Merchant guilds/houses.

What I'm most interested in though is the background culture - is it standard pseudo-medieval Europe or something else (the biggest port city IMC is Bishnagar with a distinctly 'Middle Eastern' flavour...)

During the Medieval period, Paris was not ruled directly by the king, but was overseen from the Châtelet by a royal provost. In 1141, the king sold Paris’ principal Seine port to the river-merchants’ guild. Their coat of arms subsequently became Paris’ coat of arms. It wasn’t until 1171 that Louis VII conferred a charter upon the river-merchants’ guild, confirming its “ancient right” to monopolize the river trade.
The Parisian markets were administered by a provost of merchants. The provost governed the markets, from his headquarters in the Hôtel-de-Ville, on behalf of the guilds.
