Question about Kalamar and planar cosmologies

For example, in Greyhawk there are only 13 Death Knights, but most are VERY well hidden or have nigh impentrable Lairs. Their main guy is St. Kazagoth, a former paladin of Heironous, now a blackguard that had a demon sticked on him to make himself a half fiend (So you KNOW he's dedicated). Believe, taking down a death knight is no easy task. Even in the high Powered Forgotten Realms, there are 12 death knights that formerly served the dead god Myrkul, the Lord of the undead (not to be confused with Orcus, the REAL power behind the undead, in my mind). Mykrul though, while no longer a god, does have power so they probably still exist. Still no one, save maybe Mystra Chosen, would DARE mess with ONE Death Knight much less a bunch of them.

Ah yes,
- Jan-May: Invade Oerth yet again *yawn* I suppose I have to let some meddling adventurers think that they actually defeated me through skill once again. Man, I've got to talk with the folks at WotC about the little problem with them never letting me win like I should.
- June-July: Concoct 66 new demon types for assorted diabolical schemes and inclusion in MM II.
- Aug-Oct: Put down rebellion from Grazz't. That guy is really starting to annoy me!
- Nov: Ribbon cutting ceremony at the unveiling of the new, revamped, Sigil.
- Dec: Cross-over guest appearance in Scarred Lands (boy this one could get UGLY!)
- Jan: Dentist appointment (this one will be even UGLIER!!!!)
Booked up solid through at least this year. Nope, no time for a puny little planar backwater like Kalamar I'm afraid. My, but it's good to be the king!
By the way - Death Knight - another Fiend Folio masterpiece. Where's Nemmerle when you need him?

Beside he can always just turn the dentist into tooth to fix any problems.

Planar travel is mentioned in passing in the main setting books (some of the famous NPCs are experts on it, or have travelled to other planes).
There is also a bit on the cosmology of Kalamar, and it mentions that certain parts of Kalamar touch the different elemental planes.
Also in that chapter it's mentioned that Kalamar's planet is only half a light year from a binary star system featuring a black hole. Which implies it's set in a semi-realistic universe, although I would think they would be emersed in cosmic radiation from it
