Question about True Sight...

I would suggest not using an item, as the PCs might get it and the NPC has spells. By the rules, wish straight up works. If you feel like inventing a new spell, you have some good ideas.

That doesn't work perfectly either. What if it is a Gold Dragon trying to guide the party. What if there there is an evil PC. It is better than an unrestricted item, but there are better solutions.

Read Dust of Disappearance, don't have it in front of me, it should make you invisible to true sight. That's the ruling we made today in play. I could be wrong though.....

Is there anyway to make an item or spell that protects against True Sight? Perhaps showing an alternate "true" form or blocking it out altogether?
Nondetection. There is an item based on this spell, called "Amulet of Proof against Detection and Location". (If a divination is attempted against the wearer, the caster of the divination must succeed at a caster level check (1d20 + caster level) against a DC of 19 (as if the caster had cast nondetection on his or herself).)

If you want complete protection you will have to make a new item or spell.

Would Mind Blank work?
"Yes", according to the FAQ and the PH, "no" according to the Sage. (Read the thread on Monte's board for arguments regarding this.)

From the New Archive for Sage Responces :

Here's a Sage e-mail from Your Immortal DM, first posted on Monte Cook's boards. ( http://pub58.ezboard.com/fokayyourturnfrm14.showMessageRange?topicID=2639.topic&start=41&stop=52 )

1) Do either of the spells nondetection or mind blank (on an invisible

2) Do either of the spells nondetection or mind blank (on an invisible
character) prevent detection by true seeing?

3) Is dust of disappearance (DMG p215) the only exception to this rule if
that is the case & does it stop true seeing?

4) Does either nondetection or mind blank on a polymorphed creature prevent
seeing the "true form" by true seeing?

Non detection has a chance to foil any divination spell (the divination user has to make a caster level check or the divination fails to reveal anything about the warded subject). In the case of see invisibility, a failed check means the caster sees nothing when looking at an invisible thing.

Mind blank stops things that effect the mind, and scrying (divinations that work remotely without line of effect to the subject). Mind blank does not stop direct observation with a divination spell (unless of read the mind).
So, Nondection works to disguise spells cast on me from detect magic and Arcane Sight?
Maybe.

Detect magic isn't a divination spell, and based on the SRD spell description, nondetection can only protect against divinations. But IIRC the PH version specifically mentions "detection spells", so this may just be an oversight.

(And don't forget the caster level check.)
