In the adventurer mind frame casting purify water or even having it learned may do no good, but maybe it is esential to all mainstream clerics, paladins and druids who are city dwellers.

Dirty water all around can be solved by having a basic drain sewer system, using an incline to have water travel down and out of town.

There will be some things uncurable and some who for money or any other reason can't be cured. That's part of the city, but I don't think there would be rampant issues.

Over New Years my grandfather was showing me pictures of the house he lived in Ribbleton (Preston) it was a tiny 2up-2down with a cesspit trench running along the back of the yard carrying sewerage out to who knows where (ie the river)

Apparently the thing stunk, and to my modern sensibilities seemed entirely disgusting.

Anyway an Otyugh in the trench probably would have done wonders to keep the river clean.

Oh and Druids rock...
 

Why would an otyugh clean up the water? It still has to excrete itself and that manure might be toxic being so concentrated.

And why would druids want to "clean" the water? All that fertilizer is useful and allows life to grow. Just because the city dwellers don't want that kind of life, that is their problem. Algal blooms might kill fish (and other animals) intially, but the bloom will be consumed and fish will benefit down stream.
