Reach question

I'm sorry if that as already been post, but I had not a lot of time to check if that question has already been discusted. When you have a weapon that got reach like a great spear, can you attack an enemy creature that is two square away and your ally is in the square between you and that creature?

My other interrogation about reach is the attack of opportunity. Does a creature provoke AO when it comes straight foward to you and end is course adjacent to you?

1) You can attack the enemy and the target doesn't get any cover because creatures never give cover for melee attacks (besides your allies never give cover to your enemies even for ranged attacks).

2) Unless you have threatening reach (which some monsters have) you can't make OA against non adjacent targets even if your attacks have reach.
