Red Hand of Doom: Score 2 for Koth

Mark said:
Have Koth turn up as a zombie in the big stone lion because of his failures to bring down the PCs.

While this is tempting, it is my experience that it is exceedingly rare for a villain to be able to go toe to toe with the players, inflict a great deal of damage, and still escape. It is even rarer to do that twice. Players hate villains that can do this, and it makes further encounters with them very involving.

My basic plan is for Koth to spend most of the rest of the adventure split between what he was doing at Drellins Ferry (advance scouting and raiding), and attempting to have his forces kill the players. Having survived twice, he is also pretty much an automatic hook for me. If I want the players to go somewhere, all I have to do his hint that Koth is going to be there, and the basic desire for vengeance will do the rest.

If you keep the campaign after RHoD keep him around, but don't have him actively engage the PC's. He nearly died twice against this bunch, he should be cautious.

Instead level him, have him live through the end of his horde and after all is said and done the PC's might hear how a certain former Wyrmlord that got away is involved in some nefarious happenings.
