D&D General Reincarnate is and has always been, weird.

I do like that the spell can create ghouls, wraiths, spectres and mummies, that ought to be the standard.

Personally, I hate the spell and ban it along with Wish. If you want to do the reincarnation fantasy then you must go full full Orpheus and make a descent to the Underworld, charm/beat Death (eg wrestle him like Hercules), find the spirit, convince it to return and face more trials to escape

It’s a wonderful tool to add fantasy and role play opportunities in the game.
The result should be to add fun and challenges not to turn into grotesque situation.
 

A lot of D&D cosmologically has you be born, live, die, then go to an afterlife. Sometimes you are turned into an outsider in the afterlife.

Then boom, instant adult reincarnation spell. Are you taking over an existing body with your soul? Are you creating a new random body on the spot?

And it does not really fit Celtic or Hindu reincarnation, it is its own thing and now tied heavily into D&D druidism.
It apparently happens often enough for at least some psionicists to be particularly cognizant of it, hence the incarnation awareness power in the AD&D 2E Complete Psionics Handbook, which made you aware of a creature's past lives.
 

I cannot recall the last time we used this. Might have been in 2e days. I do recall some rather silly things happening and some feeling being hurt with some rolls.

A little surprised that it is still in 5e. I must be spell-blind to it.

Which is cosmologically weird to have alongside 2e afterlife things like evil souls turning into larva in MC8, and and the Blood War fiends wanting mortal souls to turn into fiends for their armies in Planescape, along with the whole afterlife petitioner setup of Planescape.

It is possible to syncretize such stuff, but it takes a significant effort to make such things work together.
 

It's not as expansive at the early versions, but I at least prefer to extend the table to include all playable species rather than just PHB ones.

Still not sure how age impacts reincarnation. If you're a 250 year old elf, and reincarnate into a human, do you reenact that scene from The Last Crusade where the guy picked the wrong grail?

With this spell, the character brings back a dead creature in another body, provided death occurred no more than 1 week before the casting of the spell and the subject’s soul is free and willing to return. If the subject’s soul is not willing to return, the spell does not work; therefore, subjects who want to return receive no saving throw. Since the dead creature is returning in a new body, all physical ills and afflictions are repaired. The magic of the spell creates an entirely new young adult body for the soul to inhabit from the natural elements at hand. This process requires 1 hour to complete. When the body is ready, the subject is reincarnated.
A character reincarnated recalls the majority of his former life and form. The character retains his or her Intelligence, Wisdom, and Charisma scores, as well as any class abilities or skills formerly possessed. The character’s class, base attack bonus, base save bonuses, and hit points are unchanged. Strength, Dexterity, and Constitution scores depend partly on the new body. First eliminate the character’s racial adjustments (since the character is no longer of his or her previous race) and then apply the adjustments found below. The character’s level is reduced by 1. (If the character was 1st level, his or her new Constitution score is reduced by 1.)
It’s quite possible for the change in the character’s ability scores to make it difficult for the character to pursue his or her previous character class.

Some bodies may make it impossible for the reincarnated character to use some of his or her class abilities. The reincarnated character does gain any powers or abilities associated with his new form. A humanoid reincarnated into an animal body can speak the languages it formerly knew and is a magical beast.
A wish spell can restore a reincarnated character to his or her original form.

So long as some small portion of the creature’s body still exists, it can be reincarnated, but the portion receiving the spell must have been part of the creature’s body at the time of death. The magic of the spell creates an entirely new young adult body for the soul to inhabit from the natural elements at hand. This process takes 1 hour to complete. When the body is ready, the subject is reincarnated.
A reincarnated creature recalls the majority of its former life and form. It retains any class abilities, feats, or skill ranks it formerly possessed. Its class, base attack bonus, base save bonuses, and hit points are unchanged. Strength, Dexterity, and Constitution scores depend partly on the new body. The subject’s level (or Hit Dice) is reduced by 1. If the subject was 1st level, its new Constitution score is reduced by 2. (If this reduction would put its Con at 0 or lower, it can’t be reincarnated). This level/HD loss or Constitution loss cannot be repaired by any means.
It’s possible for the change in the subject’s ability scores to make it difficult for it to pursue its previous character class. If this is the case, the subject is well advised to become a multiclass character.
For a humanoid creature, the new incarnation is determined using the following table. For nonhumanoid creatures, a similar table of creatures of the same type should be created.
A creature that has been turned into an undead creature or killed by a death effect can’t be returned to life by this spell. Constructs, elementals, outsiders, and undead creatures can’t be reincarnated. The spell cannot bring back a creature who has died of old age.

The reincarnated creature gains all abilities associated with its new form, including forms of movement and speeds, natural armor, natural attacks, extraordinary abilities, and the like, but it doesn’t automatically speak the language of the new form.
A wish or a miracle spell can restore a reincarnated character to his or her original form.
Material Component: Rare oils and unguents worth a total of least 1,000 gp, spread over the remains.
 


Reincarnate is very quintessentially part of the old "Hardcore Mode Roguelike" style of D&D. You roll up your character and see how far you can get this time, and there's perma-death but with expensive respawns, only here's a budget option with a chance to backfire. Maybe your PC becomes unplayable, maybe they come out of it stronger, you don't know! Do you want to roll the dice?

It's still around because it's a tradition now, but it's kind of out of place.
But that's my favorite part of D&D; the part where it generates weird results nobody was expecting.

All things being equal, I'd rather have reincarnate as the only "bring back to life" option, and remove the raise dead/resurrection spells.
