Requesting anyone creative

How about

1) Limb/body feature gets a mind of its own (think Ash's hand in Evil Dead 2). Note, it might get actions of its own. Also think of the posibilites for ears, individual fingers, ah "Other" body parts...

2) Additional head starts growing with different personality

3) Some real things don't "exist" for the character, he can walk through some walls, creatures etc... They can't affect one another and neither knows of the other's existance. Of course there are "unreal" things that do, forcing the char to fight/evade things that aren't there for the other party members.

4) Colors are vastly different for character. "Naw, that's a Green Dragon, not a black". Note this could also be "real" for the character.

5) Character's maddness can infect others.

Extreme Dizziness - The character feels the ground they are walking on is constantly shifting. Anytime the character tries to move more than half their movement in a round, they must make a Balance check. You can move 1 foot per point of check before you fall down.
 

OH YEAH! I can get into this one. In Darksun there is an area where mutations happen if any bloodshed occurs. Nasty considering it is surrounded by about 1/2 thick ring of brambles and thorn bushes.
In that game at the Pristine Tower the mutations are designed to "improve" the survivability of a character. Of course as a DM you treat this like a "wish" and twist it often and hard. The book "Amber Enchantress" introduces this. Here a dehydrated character grows leaves and his feet literally take root like a tree to draw out moisture from the ground. So- here is some of the list of things I used based on my memory-

Intangible- Includes weapons and the clothes you wear. Can't be hurt but you reveal to the world a lot about yourself.

Armor plated healing- The injury recieved (and any for that day in your world) heal with a metal crusted plate. Can be very heavy and awkward thus reducing your DEX and spell casting but if DM determines it to be sufficent enough injuries- increased AC

Grow extra eyes- Increase Spot skills BUT who knows where that eye(s) will appear and how people will take to it

Detect life- Danmed overwhelming in a city and can't be shutoff. The version I used included plant life. Really freak out players (if they roleplay well) by having them see bacteria and the such. So small but so many

Visions of Grandeur- They see small things into the future but once they get into using it for big battles give them false readings

Magic dead zone .....on a wizard. Walking 0 level character target

Talk to animals- Can't turn it off but suddenly you can't understand teammates either as you chatter at them

Super Magnetic- Hope everyone is holding onto their sharp pointed ferious weapons well

Here's one since they EXPECT a mutation....nothing at all. Give this to someone whom is willing to jump off a cliff to see if he now flys. Oh...this message is evil.....

I say this with a smile that only a GM could ever have....well maybe the Grinch too.
 

Thank you all very much, I now have 63 different madness effects, so any more you can contribute are still welcome. I just wanted to let you all know this helps me out a ton.
 

How about Monty Python for inspiration:

Any number intended to be said by the character is replaced with another (or all numbers are multipled by a certain amount)

Any time a particular word is spoken within earshot, the character takes damage.

A particular word causes the character to place a bag over his head and withdraw from all social interactions (effective Charisma of 0?) Removal of this condition requries everyone singing some obscure song.

Character acquires a funny walk (- 5 to 20 to speed)

Any reference to certain common objects comes out as sexual innuendo.

Character refers to everyone by the same name.

Right:

-The character can suddenly see a new color that noone else sees.

Let's call it Duwen. And say that, I don't know, the sky appears duwen at noon and that horses are generally spotted duwen.

Man I miss Zimm.

-The character becomes allergic to the color blue, and breaks out in hives whenever he or she touches anythin blue. Let alone eats it.

Watch them try to figure that one out.

-The characters' hair becomes incredibly attractive to insects. They don't bite it or eat it. Just come for miles to fight for the right to sit on it.

-The character's fingernails become impervious to metal. This could be potentially useful, but just wait till the inability to cut the nails comes into play.

That one's slow, but nasty.

-Synaesthesia is nasty but hard to play. That's where the wires from different sense get crossed, so that the character hears sound as color.

There's a character in Top 10 who has this ability and does things like see high pitched sound as ultraviolet and hear perfumes named joy as Beethoven's Ninth Symphony.

Really cool, and guaranteed to drive characters crazy.
