on the first, the binding fell apart in the middle of the game, scattering pages everywhere.

My buddy had this happen with his 4E PHB. The circumstances were different, though; he left it on the window of his car on a sunny day for several hours, and only the first fifty pages or so fell out. He never tried to return of exchange it; he figured the glue had melted in the sun. But it's always been a nuisance not being able to use his book without watching for loose pages.

Mind you, we do character building by the online builder now, so we rarely open the PHBs...

I had a Savage Worlds Explorer's Edition 1st printing fall apart on me as soon as I used it. Pinnacle replaced it quickly.

There was another book that had problems, and the publisher replaced it promptly, too. I think it was SJGames, but I don't remember what book it was (a GURPS 3e softcover, I think).

Then there's the copy of Atlas Games' Penumbra Fantasy Bestiary -- the first 32 pages (IIRC; it was one printing "signature") were upside down and from later in the book. The rest of the book was fine -- that copy just had an extra copy of one chunk in the front, upside down. I didn't bother returning it.

The first copy of the GURPS Basic (3e) I had fell apart -- the pages all came loose from the cover. However, that was after years of fairly thorough use, including it falling from a moving car (35-45 mph, if I remember where it fell off correctly). I reinforced the spine of the cover with duct tape, and it was (and is) fine -- it was just more of a folder than a book.

I returned a miniatures wargaming rulebook recently when I realized I hadn't lost the one I was replacing.

I stopped by the Paizo booth at Gencon a few years ago with a flipmat that had some lamination/bubbling issues and Lisa Stevens looked it over and immediately replaced it for me from booth stock, no questions asked other than how my gaming was going and if I was having fun. I said, "Yes." :)

The binding of my copy of Midnight broke as soon as I had reached page 10. As FFG had a booth at the Spiel in Essen that year I just gave it to them. They had no copies available but about a week later I received a replacement copy.

I considered returning my copy of Ptolus, which I had bought via Malhavoc's pre-order deal, because of a dent on the spine. WW, who handled the distribution, made me a bad offer, though: they wanted me to send the book back across the pond, evaluate the damage and - maybe! - send me a replacement copy. Faced with the prospect of having to pay for international shipping, if they should decline to replace it, and the distinct possibility that there might be no copies left, I decided to live with my damaged copy.

I had some issues with the early D&D 4E books (DMG1 and Adventurer's Vault in particular.) Unfortunately, I found the customer service from WoTC at the time to be less than helpful. Thankfully, the local store where I purchased the books was gracious enough to allow me to exchange them for something else.

While the product was not a book, I did have a problem with one of the Cthulu Shot Glasses I ordered having a small crack in it. While the crack was small, I had some concerns about glass shards chipping off while trying to drink out of it. Steve Jackson Games was extremely helpful; I received a replacement a few days later. One of the employees contacted me again after the replacement was sent to ensure I had no further issues with the product. It made me feel pretty good about being a customer and buying from them.

A while back (at least 4-5 years), I had purchased a Mongoose Publishing book. My memory is a bit fuzzy, bit I believe it was Ultimate NPC. The copy I received had a problem in which some of the name tables were not legible. The best way I can describe it is to say that some of the words and letters had shadows. It was the only time I had any problems with any of their products; they were extremely helpful. I was glad to have such a positive experience because I remember it being one of the first times I had bought a third party product for D&D; I was a little unsure about buying from someone other than WoTC. The experience was positive enough that I bought several more of their products.

As far as I can recall, I've only ever returned two books, both to my FLGS. Both were replaced without any hassles.

The first of these was the 3e "Oriental Adventures". The map had been printed or folded incorrectly, and so was ruined (it couldn't be unfolded without destroying it).

The second was the "Pathfinder Campaign Setting" (the original version), which I believe had two of the signatures out of order (or one was upside down, or something). I actually got lucky with that one - the day before I discovered the fault I'd dropped the book and destroyed the spine. That one was my own stupid fault, so I wouldn't have felt right returning it... but since there was also a genuine problem with the book, I was okay. :)
