Rules for Insanity?

Swap powers for insane versions of the same. They should still be beneficial to the character but they should come with awkward twists and strange flavor text.

You could call it daily, encounter, and at will insanity. First the character becomes insane once per day, then ~every encounter, then even more often.

Example:

Brute Strike Fighter Attack 1
Wide eyed and flinging you start swinging at ghosts
Daily * Martial, Reliable, Weapon, Paranoia
Target: One creature randomly selected
Attack: Strength vs. AC
Hit: 3[W] + Strength modifier damage.

Magic Missile Wizard Attack 1
Cackling loudly you start fires in the vicinity.
At-Will * Arcane, Fire, Implement, Pyromania
Target: One creature and one object.
Attack: Intelligence vs. Reflex.
Hit: Yada yada
Special: This counts as basic attack... snip! And you must pick a nearby object to try to set alight.
